Phaware debuts Heart Works app to empower the global PH community
Mobile hub offers AI support, health tracking, and research tools for patients

Global advocacy group Phaware has launched a new app, called Heart Works – phaware, to provide a centralized hub for education, health tracking, and community connection for those living with pulmonary hypertension (PH).
The app is designed to be a comprehensive tool for patients, caregivers, and medical professionals. By bringing together research and educational resources into a single mobile platform, the organization hopes to reduce the isolation often felt by those living with the rare lung disease.
A global call to action
“The Heart Works app is a global resource for the pulmonary hypertension (PH) community,” Steve Van Wormer, co-founder and president of the Phaware Global Association, said in a statement to Pulmonary Hypertension News. “From Burbank to Barcelona, Buenos Aires to Beijing, we’re breaking down isolation by bringing together the very best resources, not only for patients and caregivers, but for medical professionals as well.”

The new app can be used on iPhone, Apple Watch, and Android devices, according to a press release from Phaware. The app, created in partnership with Pulmonary Hypertension News and several other PH-focused organizations, is now available on the App Store and Google Play.
Phaware launched the app in time for this year’s Rare Disease Day, a day of awareness observed globally every year on the last day of February.
“Heart Works is more than a mobile app, it’s a global call to action,” Van Wormer said. “On Rare Disease Day, we celebrate the strength of the PH community by launching a platform that puts connection, education, and research directly in patients’ hands. This patient-powered research model accelerates discovery while making participation more accessible than ever.”
The new app features a variety of tools to help people with PH track and manage their health. For example, it allows patients to conduct assessments of exercise capacity at home, participate in clinical trials remotely, and complete surveys to measure their quality of life.
The app is also loaded with educational materials, including podcasts, interviews with medical experts, and global daily news feeds. There’s also an AI-powered chatbot specifically tailored to provide information about PH.
“One of the core features of Heart Works is the phaware Chatbot, which allows users to tap into a curated knowledge base designed to answer real-world questions based on real-life patient lived experiences,” Van Wormer said.
In addition to resources for people living with PH, the app offers a range of resources for healthcare providers treating the disease.
“Now available in every country with a recognized PH advocacy group, the Heart Works helps individuals and healthcare providers stay informed, connected, and better equipped to navigate the PH journey no matter where they live,” Van Wormer said.
